Complete Process to Make an Ayushman Bharat (PM-JAY) Card

The Ayushman Bharat Pradhan Mantri Jan Arogya Yojana (AB PM-JAY) provides cashless health insurance coverage of up to ₹5 lakh per family per year for eligible beneficiaries at empanelled government and private hospitals.


Step 1: Check Your Eligibility

Before applying, verify whether your family is eligible for the scheme.

How to Check

  1. Visit the Ayushman Bharat Beneficiary Portal.
  2. Click “Am I Eligible” or log in using your mobile number.
  3. Search using any one of the following:
    • Aadhaar Number
    • Mobile Number
    • Family ID (where applicable)
    • Ration Card (in some states)

Step 2: Keep the Required Documents Ready

Prepare the following documents before starting the application:

  • Aadhaar Card
  • Mobile number linked with Aadhaar
  • Ration Card (if applicable)
  • Family ID/State ID (where applicable)
  • Passport-size photograph (if requested)

Step 3: Log In to the Beneficiary Portal

  1. Open the official Ayushman Bharat Beneficiary Portal.
  2. Enter your mobile number.
  3. Verify your identity using the OTP sent to your phone.
  4. Search for your family’s record.

Step 4: Verify Family Details

If your family’s name appears in the records:

  1. Review all family member details carefully.
  2. Select the member whose Ayushman Card you want to generate.
  3. Proceed with e-KYC.

Step 5: Complete e-KYC

Complete identity verification through Aadhaar authentication.

The portal may require:

  • Aadhaar OTP verification
  • Biometric authentication (where applicable)
  • Live photograph (if required)
  • Identity verification

Step 6: Submit the Application

After successful verification:

  1. Review the entered details.
  2. Submit the application.
  3. Your request will be sent for approval.

Step 7: Download the Ayushman Card

Once your application is approved:

  1. Log in to the Beneficiary Portal again.
  2. Click Download Ayushman Card.
  3. Save the PDF or print a copy for future use.

Offline Process

If you are unable to apply online, visit any of the following with your Aadhaar Card:

  • Common Service Centre (CSC)
  • Empanelled Government Hospital
  • Ayushman Mitra Help Desk

The operator will:

  • Check your eligibility.
  • Complete Aadhaar verification.
  • Capture your photograph (if required).
  • Generate and print your Ayushman Card (where available).

Important Points

  • There is no registration fee for eligible beneficiaries.
  • PM-JAY is an eligibility-based scheme. Only families covered under PM-JAY or an eligible state scheme can generate an Ayushman Card.
  • Cashless treatment is available only at empanelled hospitals.
  • The Ayushman Card is different from an ABHA Health ID and serves a different purpose.
